    Solid book that shows that there is no "one" root cause to issues, but generally a number of issues that accumulate to become the root cause. Good examples from flight, medical, and nuclear reactor incidents that are easily applied to software systems. That said, the content is dense and I had take regular breaks to give my brain a rest.     The ideas in the book are very interesting, and the content is clearly well researched, but the writing style is very dense. This is not the most approachable book about the topic, but if you stay with it you will learn quite a lot about how failures and safety in complex systems.
